If you are volunteering with the City of Mississauga and Councillor Horneck, you must complete the following waivers.  You may either use our electronic signature system and do everything online or you can print out the below forms and either photograph them and email them to william.pitcher@mississauga.ca OR bring physical copies to the volunteer activity you are assisting.

If you are under 18 years old, the forms must also be signed by a parent or legal guardian.
